Daily Wellness Screening

All Wake Forest students, faculty, and staff will be expected to complete a daily wellness screening using the SneezSafe web-based symptom screening tool. This screening will help promote health and safety on our campus by monitoring symptoms and overall wellness of the Wake Forest community. Developed in partnership with Wake Forest Baptist Health, the results of these screenings are confidential.

Students who will be residing in university housing or accessing campus should log onto the site to register at least one day prior to arrival and start using this screening tool the day they arrive on campus. Faculty and staff should start completing their daily wellness screenings immediately, regardless of work location or status. The entire campus community is expected to complete the daily screening receive clearance from SneezSafe before proceeding to campus or leaving living quarters.

Faculty & Staff Registration

Register on the SneezSafe website using the following information: 

  • Employee registration code WF1
  • Employee ID number (on Deacon OneCard)

Faculty and staff are encouraged to provide a cell phone number upon registration and select the option to receive text reminders to complete the daily screening.

The Faculty and Staff Clinic will have access to the daily screening results to assist employees who need a medical assessment. Personal information will be kept confidential. Based on symptoms and/or known exposures to COVID-19, the daily screening will let faculty and staff know if they should contact the Faculty and Staff Clinic for additional information or testing.

Student Registration

Register on the SneezSafe website using the following information: 

  • Student registration codes:
    • Off-Campus Students: WF2A
    • On-Campus Students: WF2B
  • Student ID number

Students should provide a cell phone number upon registration and must select the option to receive text reminders to complete the daily screening. 

The Student Health Service will have access to the daily screening results to assist students who need a medical assessment. Personal information will be kept confidential. Based on symptoms and/or known exposures to COVID-19, the daily screening will let students know if they should contact Student Health Service for additional information or testing.

COVID-19 Community Research Partnership

Researchers at Wake Forest Baptist Health have partnered with Wake Forest University to better understand the spread of COVID-19. Study participants complete short digital questionnaires over several months about possible exposures, symptoms and health care visits. Learn More »

Many Wake Forest students, faculty, and staff are already participating in this research study. Participants now have the ability to complete the research study questionnaire AND the daily wellness screening using SneezSafe. If you already participate in the research study and would like to begin completing your questionnaire via SneezSafe or would like to volunteer to join the research study, follow the instructions below based on your registration status: 

Participating in the research study is completely optional. As a reminder, any information submitted to the research study is confidential and will only be shared with the researchers. If you have any questions about SneezSafe or have any technical issues, please email contact@nullsneezsafe.com.

  • What is SneezSafe?

    SneezSafe is a web-based symptom screening tool, developed in partnership with Wake Forest Baptist Health, to monitor potential COVID-19 exposure and symptoms of the Wake Forest community per recommendations of the CDC and local public health officials.

  • How does SneezSafe relate to the university’s Wellness Screening Policy?

    The Wellness Screening Policy states that all members of the Wake Forest community (faculty, staff, and students) will be expected to complete a daily screening for symptoms and exposure to COVID-19. SneezSafe is the web-based screening tool the campus community will be expected to use.

  • I can’t find SneezSafe in the app store. How do I download it?

    SneezSafe is a web application tool that is accessed by visiting survey.sneezapp.com or by clicking the link in a daily text reminder. SneezeSafe is not a mobile app and you will not need to download anything in order to use it. SneezeSafe should not be confused with the Sneez mobile app.

  • Who should complete the SneezSafe daily wellness screenings?

    All faculty, staff, and students are expected to complete the daily wellness screenings. Faculty and staff members should complete the daily wellness screenings regardless of their work location. Participation in the daily wellness screening is an opportunity for our campus community to proactively monitor exposure and symptoms, helping to keep the broader community healthy.

  • When should I start completing the SneezSafe daily wellness screenings?

    All faculty, staff, and students should register for SneezSafe and begin completing daily wellness screenings immediately.

  • Do I need to complete the SneezSafe daily wellness screenings every day?

    Yes, all faculty, staff, and students should complete the daily wellness screenings every day, including days they do not intend to be on campus and on weekends.

  • When should I take the SneezSafe daily wellness screening?

    Faculty and staff arriving on campus before 8:00am should complete their screening by 4:30pm the day before. If arriving between 8:00am and 4:30pm they should complete their screening before proceeding to campus. If arriving after 4:30pm they should complete their screening before 4:30pm. Students should complete their daily wellness screening when they receive their text reminder.

  • Can I customize my reminder time or receive an email instead?

    Currently SneezSafe is not able to customize reminder times based on individual preferences. However, participants can choose to receive text OR email reminders. If you are not receiving SneezSafe reminders but would like to, or if you would like to change your text or email preference, please submit a SneezSafe Survey Reminders Request. You may be contacted with further instruction via email, so be sure to provide a valid email that you check regularly. Please allow 2-3 days for your request to be processed.

  • What if I don’t have access to a smartphone or computer?

    Faculty and staff who do not have access to a smart phone or computer should contact Human Resources at (336) 758-4700.

  • Who has access to the information I submit through SneezSafe?

    Faculty and staff responses will only be seen by the medical professionals at the Faculty and Staff Clinic, operated by Wake Forest Baptist Health. WFU Human Resources will only receive a list of those who completed the screening to ensure they were cleared to be on-site. Student responses will only be seen by the medical professionals in Student Health Service.

  • When I try to log into SneezSafe, the browser won’t load. What do I do?

    SneezSafe has identified an issue in which browser caching may cause access issues. If this happens, participants are encouraged to (1) take the survey by clicking a link from a previous text, (2) visit survey.sneezsafe.com > Daily Wellness Survey and have a new text sent to you by clicking on “I don’t remember, please text me” or (3) use a different browser or device.

Research Study

Those who opt into the COVID-19 Community Research Partnership Study will be required to answer a question about their sex assigned at birth. Participation in this study is completely voluntary and separate from the requirement to complete the SneezSafe daily wellness screening. Demographic information, including sex assigned at birth, is only shared with the study researchers, not Human Resources.
